Vial strength
Printed on the vial label, e.g. "10 mg/mL". Common tirzepatide strengths: 5, 10, 15, 20, 30 mg/mL.
Your dose
Exactly as written on your prescription. 1 mg is 1,000 mcg.
- Dose

- Concentration

- Exact volume
- 0.25 mL
- 25 units (1 unit = 0.01 mL on a U-100 syringe)
| Weekly dose | When | Units | mL |
|---|---|---|---|
| 2.5 mg | Weeks 1–4 (starting dose) | 25 units | 0.25 mL |
| 5 mg | Week 5 on | 50 units (over capacity) | 0.5 mL |
| 7.5 mg | After 4+ weeks at 5 mg | 75 units (over capacity) | 0.75 mL |
| 10 mg | After 4+ weeks at 7.5 mg | 100 units (over capacity) | 1 mL |
| 12.5 mg | After 4+ weeks at 10 mg | 125 units (over capacity) | 1.25 mL |
| 15 mg | After 4+ weeks at 12.5 mg (maximum) | 150 units (over capacity) | 1.5 mL |
How do you convert mg to units?
Divide the tirzepatide dose in mg by the vial's mg/mL and multiply by 100. At 10 mg/mL, 2.5 mg is 25 units and 7.5 mg is 75 units. 12.5 and 15 mg need more than 1 mL at that strength, so they are usually given from stronger vials: at 20 mg/mL they are 62.5 and 75 units.
units = dose (mg) ÷ concentration (mg/mL) × 100This page uses the same method as the GLP-1 Dosage Calculator, which explains each term and assumption in full.
| Weekly dose | When | Units | Volume |
|---|---|---|---|
| 2.5 mg | Weeks 1–4 (starting dose) | 25 units | 0.25 mL |
| 5 mg | Week 5 on | 50 units | 0.5 mL |
| 7.5 mg | After 4+ weeks at 5 mg | 75 units | 0.75 mL |
| 10 mg | After 4+ weeks at 7.5 mg | 100 units | 1 mL |
| 12.5 mg | After 4+ weeks at 10 mg | 125 units | 1.25 mL |
| 15 mg | After 4+ weeks at 12.5 mg (maximum) | 150 units | 1.5 mL |
Worked example
A 2.5 mg tirzepatide dose from a 10 mg vial mixed with 1 mL of bacteriostatic water.
- 1Concentration: 10 mg ÷ 1 mL = 10 mg/mL
- 2Volume: 2.5 mg ÷ 10 mg/mL = 0.25 mL
- 3Units: 0.25 mL × 100 = 25 units
- 4Doses per vial: 10 mg ÷ 2.5 mg = 4
Draw to the 25 units line, on a 0.3 mL (30 units) U-100 insulin syringe.
Frequently asked questions
How many units is 2.5 mg of tirzepatide?
50 units at 5 mg/mL, 25 units at 10 mg/mL, 16.7 units at 15 mg/mL, 12.5 units at 20 mg/mL, 8.3 units at 30 mg/mL. See 2.5 mg in units for the syringe to use at each strength.
How many units is a Zepbound single-dose vial?
Every Zepbound single-dose vial holds 0.5 mL, whatever its strength, so the whole vial is the 50-unit line on a U-100 syringe: 2.5 mg is 5 mg/mL, 15 mg is 30 mg/mL. Follow the Instructions for Use that come with the vial.
What is the tirzepatide dose schedule?
The Zepbound label starts at 2.5 mg once a week for four weeks, then 5 mg. Doses can rise in 2.5 mg steps after at least four weeks on each, to maintenance doses of 5, 10 or 15 mg; 15 mg is the maximum. Your prescriber sets your schedule.
